Khan Academy -- Videos and practices for math and other subjects.  This site has come highly recommended to me by various students.
MathsIsFun.com -- Examples and practice.
Regents Exam Prep Center -- The Oswego City School District of New York's site with review material for the Regents Exam.  Covers topics in pre-Algebra, Algebra, Geometry, Algebra 2/Trigonometry, Global History, US History, Earth Science, Chemistry, Physics, English, and study strategies.  Also includes a reference library.
Virtual Nerd - "Over 1,500 video lessons covering Middle Grades Math through Algebra 2."
